What is Dupuytren’s contracture?

Dupuytren’s contracture is a degenerative disorder in which the ligamentous structures in the palm of the hand stiffen and shorten, leading to curling of the fingers inward and restriction of their mobility to extension. It is a disease in which the ligamentous bands in the palm of the hand become stiff and shortened, limiting the extension of the fingers – most commonly the ring and little fingers. The cause is not always clear, but genetics, diabetes, smoking or repetitive physical stress play a role. Gradually, hand function is limited and the quality of life is significantly reduced. Surgery can significantly improve hand function and slow the progress of the disease.

Consultation with a doctor

  • Diagnosis usually based on visual inspection and palpation alone – a stiff band in the palm and inability to straighten the fingers are typical.
  • Examination of the extent of disability and assessment of treatment options

Preparation before the procedure

  • Discontinue or adjust the dosage of blood thinners 7 days before the procedure as agreed with the doctor
  • Have a light meal 2–3 hours before the procedure
  • Do not wear jewellery or nail polish in the area

Procedure

  • An outpatient procedure that loosens or removes excess ligament
  • The procedure takes 30–60 minutes
  • During the procedure, the blood supply to the arm is temporarily restricted to minimise bleeding

After the procedure

  • Immediate rehabilitation is important – from day 2
  • Passive and later active exercises
  • Hand fixation at night (3–6 months)

  • Return to normal depends on the profession – usually 1–3 months
  • The need for regular exercise to maintain the outcome
  • Night fixation of the hand with a splint to prevent the return of contracture for 3–6 months

The treatment is not suitable for people:

  • With acute infection at the site of surgery
  • With severe general disease without stabilisation
  • Allergic to anaesthetics or disinfectants

Possible complications are rare and may include:

  • Bleeding, infections, nerve or tendon injuries
  • Scarring and re-limitation of mobility without rehabilitation
  • Disease recurrence – even after surgery, the condition can reappear
